F2F documentation requirements clarification in MLN article

Yesterday, CMS released a Special Edition MLN article to clarify documentation requirements for the HH F2F encounter. The article reiterates that diagnoses alone do not support the need for skilled services and that standard language such as “taxing effort” is not acceptable to support that the patient is homebound.   The article includes some great examples of acceptable and unacceptable encounter documentation.
